Executive Director

Nelson Rodrigues

With nearly two decades of experience in the senior living industry, Nelson has dedicated his career to creating compassionate, vibrant communities where residents and staff thrive. He began his journey as an Executive Assistant at the Terraces of Los Gatos, where he spent over eight years gaining invaluable insight into senior living operations. He then served as the Executive Director of Sunny View Retirement Community, leading a large team and enriching the lives of residents for six impactful years.

A graduate of San Jose State University, Nelson earned his Bachelor’s degree in Journalism in 2004. He became a certified Resident Care Facility for the Elderly (RCFE) Administrator in 2006 and a Nursing Home Administrator in 2013, reflecting his dedication to professional growth and excellence in senior care.

His commitment to this field is deeply personal, inspired by his maternal grandfather, Manuel, who battled Alzheimer’s in Pico, Azores, Portugal, where care resources were scarce. In his honor, Nelson has devoted his life to fostering environments where older adults are treated with dignity, respect, and compassion.

Senior Living Consultant

Nathaniel Tan

Nathaniel joined Agemark in April 2023 as a Sales and Marketing Assistant and has since transitioned to his current role as a Senior Living Consultant. He chose a career in senior living because he wanted a job that feels meaningful and allows him to be around happy people. Nathaniel finds fulfillment in connecting with families and providing them with peace of mind, knowing their loved ones are in good hands. Having personally experienced caring for a loved one, he understands the challenges involved and is dedicated to making the process easier for everyone.

Nathaniel chose to join Agemark because of the remarkable leadership and unwavering support from everyone in the company. He believes that the exceptional leaders at Agemark provide the backing and guidance necessary for him to grow professionally. Nathaniel loves helping seniors discover TreVista Concord as their new home, where they can thrive in a supportive and caring environment.

Originally from Hercules, CA, Nathaniel enjoys motorcycling and volunteering in schools that lack access to technology, such as Computers for Youth. Before joining Agemark, he worked as a Market Retail Manager at Microsoft in Seattle for two years and as an Assistant Sales Manager at Best Buy, later moving to the Geek Squad for another two years.

Director of Nursing

Stephanie Perez

Stephanie has dedicated her career to making a positive impact in the lives of the elderly, ensuring they maintain their dignity and health. Starting as a Wellness Assistant, she quickly advanced to Memory Care Manager and now the Director of Nursing, where her passion and leadership shine every day.

A graduate of Los Medanos College, Stephanie holds a Licensed Vocational Nurse (LVN) certification and an Associate’s degree. Her education spans multiple disciplines, including Liberal Arts in Arts and Humanities, Behavioral and Social Sciences, and Math and Science. This diverse academic background complements her extensive hands-on experience in senior care.

What Stephanie finds most rewarding about her role is connecting with residents and seeing their smiles each day. Knowing she plays a part in helping them maintain a healthy and fulfilling life brings her immense joy and purpose. She is committed to fostering a positive, nurturing environment where residents feel valued and supported in every aspect of their care.
