TreVista in Concord is being sold by TreVista Concord LP to CTR Partnership. The sale is currently under contract, with an anticipated closing date of February 28, 2025.
This sale means that Agemark will no longer manage TreVista. Kalesta Healthcare Group will be the community’s new manager. Services will continue as usual until the close date, and then Kalesta will support the facility and new operations. The building’s name will change, but there are no plans to change the services provided to each resident. The new operator’s goal will be to provide the current and future residents with an experience that everyone in the community will be proud of.
Over the last six years, current community leadership has seen the completion of an extensive renovation, managed operations during a global pandemic, and, most notably, had the opportunity to serve hundreds of seniors and their families while supporting an exceptional team of employees.
Kalesta is a healthcare support center located in Rocklin, CA, with the mission to Uplift, Nurture, and Heal everyone they serve. The Kalesta team consists of experienced and qualified team members in the senior care sector and provides support to Assisted Living Facilities and Skilled Nursing Facilities within Northern California. The announcement was made on January 23, 2025 through in-person meetings and written communication provided to the community’s staff, residents and resident family members.
